Medical Examination Processing

Tashkent International Clinic provides medical examinations for applicants applying for a visa to Canadian Immigration and Citizenship (CIC). See the instructions below on how to make an appointment, how to proceed with payment, and a list of documents requested for the medical examination.

Before making an appointment at the Tashkent International Clinic for a medical examination, registration and payments must be submitted at the website: www.intms.org

Note: Before you visit website: www.intms.org for registration and for payment, you will need the following list of documents:

  • Recent Electronic Photo 35 x 45 mm to upload (in color, white background, taken within the last six months), For information on photo requirements, please visit: www.canada.ca

  • Passport (Red or traveling) to complete personal information

  • E-mail address

  • Payment: MasterCard or Visa credit card

Below are instructions on how to fill out the form on the website: www.intms.org, please follow step-by-step instructions:

Payment

  • Register or login at www.intms.org with your e-mail address and personal password.

  • Read the “Terms of Agreement” carefully. If you agree to the terms for the provision of personal information, please check the box (√) and proceed to the next step.

After filling out the form correctly, you will be redirected to the payment portal for International Medical Services, which will automatically calculate the costs for both the individual and family medical examinations.

Please note: Tashkent International Clinic does not charge for medical examination, the payment is sent to the service of International Medical Services.

The non-refundable cost of a medical exam and all required lab analysis is:

  • Applicants 0 through 10 years old – 105 USD

  • Applicants 11 years old through 14 years old – 145 USD

  • Applicants 15 years old and older – 230 USD

At this stage, international bank card – Visa or MasterCard is required:

  • Make sure the name and last name of cardholder, card number and expiration date are correct

  • Make sure the card has sufficient funds on it

  • After payment is successful, the receipt and the information you filled in will be automatically sent to the email address of the Tashkent International Clinic, as well as to the email address that you indicated when filling out the form

  • If the payment request is rejected, contact the bank provided the card to clarify the reason for the payment denial

  • Payment is non-refundable

Scheduling your appointment for a medical exam

After you have received confirmation of payment, call the Tashkent International Clinic to schedule your appointment. The phone number is: +998 78 120 11 20. Please call morning time from 8:00 a.m. till 12:00 p.m. and afternoon time from 2:00 p.m. till 5:00 p.m.

The day of your appointment

On the day of the medical examination please arrive STRICTLY AT THE APPOINTED TIME (not late or early). Fasting is not required.

Please prepare and bring list of required documents for medical examination:

  • Valid traveling (red) passport and, for each family member if you are traveling with your family

  • 2 copy of traveling (red) passport (main page with photo) for each family member

  • Medical Report (IMM 1017) with your IME # or if you do not have an IME # then your IMM 1017B UPFRONT document

  • All current and past medical and Vaccination records (If you have the above listed documents available, please bring them. If you do not have the documents listed above, it is okay)

Terms and Conditions

You are responsible for the validity and accuracy of all the information you provide on this form.

Applicants should arrive according to the scheduled appointment time with their passport and any other documents related to their medical exam.

Applicants must abide by the rules, procedures and requirements to enter the clinic territory. The applicant will be asked to switch off their mobile phone and pass all electronic devices, including car keys, any liquid items, sprays, etc. to the security guard.

Video Recording: Please note that all activity in public areas is being monitored and recorded by clinic security cameras, including microphones recording what is being said.

Payments will be processed according to the fee schedule outlined in Step 1. Payments are non-refundable.
